Supports members with complex medical and social needs through outreach, care planning, health coaching, appointment accompaniment, service referrals, and case documentation.
What you'll do
- Manage patient caseloads, complete assessments, and develop and carry out care plans with patients and clinical teams.
- Conduct outreach, build rapport, coach members on health behaviors, and connect them with medical, social, and public services.
- Accompany members to appointments, advocate for them, and document encounters in case management systems.
What they ask for
- At least 2 years of experience with and comfort working with CalAIM populations of focus.
- Valid driver’s license, automobile insurance, and reliable transportation; minimum 75% travel required.
- High School Diploma or equivalent.
- Ability to work outdoors and independently as well as with a team; communicate effectively and use computers for case documentation.
Nice to have
- Bilingual in English and Spanish is strongly preferred.
Pay
