About the job
Supports patients with complex medical and social needs in the San Fernando Valley through outreach, care management, health education, and connections to services.
What you'll do
- Manage patient caseloads, complete assessments, develop care plans, and document encounters.
- Provide coaching and advocacy, accompany members to appointments, and connect them with local services and programs.
- Conduct outreach to eligible patients and enroll them in the Akido ECM Program.
