Supports members with complex medical and social needs through care planning, health coaching, outreach, service referrals, and AI-guided clinical assessments.
What you'll do
- Manage patient caseloads, complete assessments, develop care plans, and document encounters.
- Use Scope AI to support structured clinical assessments and diagnostic decision-making with providers.
- Coach and advocate for members, accompany them to appointments, and connect them with services and programs.
