Supports Skid Row patients with complex medical and social needs through outreach, care planning, health coaching, and connections to services.
What you'll do
- Manage patient caseloads, complete assessments, and develop and carry out care plans with patients and clinical teams.
- Provide outreach, coaching, advocacy, and appointment support; connect members to local, county, and state services.
- Document encounters and enroll eligible patients in the Akido ECM Program.
