Provides comprehensive outpatient primary care to patients across the lifespan in a value-based, team-based practice in Rhode Island.
What you'll do
- Manage a patient panel, including preventive care and chronic disease management, seeing 16–20 patients per day.
- Coordinate care with physicians and care teams, and participate in quality improvement initiatives.
- Document care and manage patient records in an EMR; provide shared after-hours call coverage for the patient panel.
