About the job
Provide outpatient primary care to a diverse patient population in Rhode Island, using team-based, value-based care and managing an assigned patient panel.
What you'll do
- Deliver comprehensive primary care, including preventive care and chronic disease management, for approximately 16–20 patients daily.
- Coordinate with physicians and advanced practice providers, and participate in care coordination and quality improvement.
- Document and manage records in the EMR; provide shared after-hours call coverage and optional in-office procedures.
