Provides hybrid, longitudinal primary care and behavioral health support for members, including chronic disease management, urgent and preventive care, care planning, and clinical guidance to an interdisciplinary team.
What you'll do
- Conduct clinical visits, follow-ups, outreach, and compliant documentation for members.
- Manage chronic and behavioral health conditions, prescribe within scope, and coordinate referrals and care plans.
- Collaborate with and guide the care team, oversee Advanced Practice Clinicians, and support complex cases.
