What you'll do
- Manage a member caseload and develop care plans with members and RN Care Managers.
- Conduct assessments and behavioral health screenings, and connect members with appropriate programs and community resources.
- Support appointments, testing, referrals, and care-team communication, including meeting members in community settings.

Pay
