Supports members with complex health needs by coordinating social care, developing care plans, connecting them to services, and collaborating with RN Care Managers and community partners.
What you'll do
- Manage a member caseload and address social, behavioral, and environmental barriers to health, including through assessments, care plans, and referrals.
- Help members access appointments, testing, and community resources, and meet them in community settings as needed.
- Document interactions and coordinate tasks and communication with members, the care team, and community partners.
