What you'll do
- Engage members, conduct screenings, and coordinate with RN Care Managers on care plans and program placement.
- Connect members with community and social services, provide non-clinical preventive care education, and follow up on care coordination needs.
- Document interactions, track tasks, and coordinate with internal and external providers and community resources.
