Supports members with complex social, behavioral, and physical needs through care coordination, community-based services, and non-clinical follow-up in partnership with RN Care Managers.
What you'll do
- Assess members, help develop and carry out care plans, and connect them with community and social services.
- Follow up on care coordination needs, provide routine non-clinical preventive-care education, and meet members in community settings as needed.
- Document interactions, track tasks, and coordinate with internal care teams and external providers.
