About the job
Provides in-person and virtual primary and wrap-around care for members of all ages, including chronic and acute care, behavioral health, and preventive services, in collaboration with an interdisciplinary team.
What you'll do
- Conduct clinical intakes, provide ongoing primary and wrap-around care, and follow up after transitions or acute needs.
- Manage chronic and behavioral health conditions, prescribe specified medications, close quality gaps, and document diagnoses and care.
- Collaborate with the care team on care plans, case conferences, referrals, and clinical programs.
