What you'll do
- Conduct clinical intake visits and provide longitudinal primary or wrap-around care, including chronic disease management and preventive care.
- Follow up and proactively outreach to members; document diagnoses and close quality gaps using appropriate codes.
- Collaborate with the care team on care plans, referrals, and clinical programs.
