Provides short-term behavioral health care, triage, and care coordination for members, including those with serious mental illness or substance use disorder.
What you'll do
- Provide behavioral health support and short-term therapeutic interventions in person and virtually; develop care plans and maintain engagement with high-acuity members.
- Educate members on preventive care, behavioral health, and mental illness; refer them to medical, social, and behavioral health resources.
- Collaborate with care teams on escalations, program placement, and transitions of care.
