Provides hybrid primary and wrap-around care, managing chronic and acute conditions, behavioral health, preventive care, and member care plans in collaboration with an interdisciplinary team.
What you'll do
- Conduct clinical intakes, provide ongoing care, and follow up after transitions or acute needs.
- Document diagnoses and quality gaps; prescribe medications for specified behavioral health conditions and refer members to specialists as appropriate.
- Collaborate with the care team on member care plans and contribute to clinical programs, processes, and policies.
