What you'll do
- Conduct telephonic, virtual, and in-person outreach; complete member intake and facilitate handoffs to care teams.
- Document member information and visit notes in Commons, track outreach tasks, and communicate urgent needs or safety concerns.
- Plan field visits and travel, build CBO referral relationships, and meet member outreach and activation targets.
