Provides whole-person primary care to complex members in Ohio, including chronic and acute care, behavioral health, prevention, care planning, and clinical oversight of Advanced Practice Clinicians.
What you'll do
- Conduct clinical visits, follow-ups, and outreach; manage chronic, acute, behavioral health, and preventive care needs.
- Collaborate with interdisciplinary teams on care plans, panel management, and complex case reviews; document care and close quality gaps.
- Coach and oversee clinical team members and Advanced Practice Clinicians, including chart reviews and supervision assignments.
