Provides primary and behavioral health care, chronic disease management, and preventive support to members while collaborating with care teams and overseeing Advanced Practice Clinicians.
What you'll do
- Conduct clinical visits, follow up with members, prescribe medications within the stated scope, and document care and diagnoses.
- Collaborate with interdisciplinary teams on case reviews, care plans, panel management, and referrals.
- Provide clinical guidance, chart reviews, and supervision of Advanced Practice Clinicians.
