About the job
Manages a panel of adult and geriatric members with chronic medical and behavioral health needs through assessment, care planning, clinical follow-up, and coordination with providers and community resources.
What you'll do
- Develop and monitor member-centered care plans, reassessing needs and coordinating clinical and social supports.
- Conduct clinical visits and follow-ups, including medication management, chronic disease support, and behavioral health screening.
- Coordinate with interdisciplinary teams, document care, and address quality gaps.
