What you'll do
- Evaluate member needs and eligibility, identify risks, and coordinate referrals and care-plan activities.
- Work with members and multidisciplinary care teams to address barriers, improve care quality, and support informed health decisions.
- Monitor care progress and document services in accordance with regulatory, accreditation, and company requirements.

- Able and willing to travel up to 75% for face-to-face member visits; reliable transportation required.
- At least two years of case management experience.
