Coordinates and monitors care plans for Medicaid members, evaluating their needs and helping them access appropriate healthcare, support, and community services.
What you'll do
- Evaluate members’ needs and eligibility, identify risks, and coordinate and monitor care plan activities.
- Collaborate with clinical staff and other programs to address care barriers, escalate quality concerns, and support appropriate services.
- Coach and educate members to encourage engagement and informed healthcare and lifestyle decisions.
