About the job
Coordinates and monitors care plans for members, evaluating their needs, connecting them with services, and supporting appropriate healthcare outcomes through field visits and member education.
What you'll do
- Evaluate member needs and eligibility, identify risks, and recommend referrals or care plan actions.
- Coordinate and monitor care plan activities; collaborate with clinical teams and present cases for multidisciplinary review.
- Support member engagement and informed healthcare decisions, escalate quality-of-care issues, and document care in line with applicable processes.

- West Virginia residency in one of the listed counties: Barbour, Brooke, Doddridge, Hancock, Harrison, Lewis, Marion, Marshall, Monongalia, Ohio, Pleasants, Ritchie, Taylor, Tyler, Upshur, or Wetzel.
- Reliable transportation and ability and willingness to travel at least 50% in the assigned region.
- At least 2 years’ experience with personal computers, multi-system navigation, and MS Office Suite applications.
Nice to have
- Medicaid, waiver, foster care, crisis intervention, managed care/utilization review, case management, or discharge planning experience.
- Familiarity with QuickBase.

$21.10 - $36.78 USD base hourly rate
