Coordinates care for members by assessing needs, providing education and support, and collaborating with healthcare teams to promote appropriate outcomes.
What you'll do
- Coordinate member care, provide coaching and education, and help members make informed healthcare and lifestyle choices.
- Present cases for multidisciplinary review and work with care teams to address barriers to goals.
- Evaluate referred members’ needs and eligibility, recommend case resolutions, and escalate quality-of-care issues.
