About the job
Provides behavioral health case management for members by assessing needs, coordinating care and community services, and supporting effective care transitions and outcomes.
What you'll do
- Conduct behavioral health and psychosocial assessments; develop and monitor individualized care plans.
- Coordinate care among behavioral health, medical, community providers, and interdisciplinary teams; support transitions of care.
- Advocate for members, connect them with community resources, and address barriers to progress.

Pay
$54,095.00 - $116,760.00 USD base salary
