About the job
Provides care management and evidence-based short-term psychotherapy for patients with behavioral health needs, coordinating with primary care teams and a psychiatric consultant.

- Screen and assess patients for common mental health and substance use disorders; develop care plans and referrals.
- Provide short-term counseling and evidence-based treatments, and coordinate care with primary care teams and a psychiatric consultant.
- Document patient encounters and support regional and organizational behavioral health program efforts.
