About the job
Provides care management and evidence-based short-term psychotherapy for patients with behavioral health needs, in collaboration with primary care teams and a psychiatric consultant.
What you'll do
- Screen and assess patients for common mental health and substance use disorders; develop care plans and arrange referrals.
- Provide short-term counseling and evidence-based treatments, coordinating care with psychiatric consultants and primary care teams.
- Document patient encounters and support regional and organizational behavioral health program efforts.
