Provides care management and evidence-based short-term psychotherapy for patients with behavioral health needs, coordinating care with primary care teams, a psychiatric consultant, and external resources.

- Screen and assess patients for common mental health and substance use disorders; develop care plans and referrals.
- Provide short-term counseling and evidence-based treatments, coordinating with the psychiatric consultant and care teams.
- Document patient encounters and counseling sessions, and support regional behavioral health program efforts.
