About the job
Coordinates care for Aetna LTSS members by assessing needs, arranging services, and monitoring care plans through face-to-face field visits.
What you'll do
- Evaluate member needs and eligibility, identify risks, and recommend services or referrals.
- Coordinate care-plan activities, monitor progress, and collaborate with clinical staff to address barriers and quality concerns.
- Coach and support members in making informed healthcare and healthy-lifestyle decisions; document care in accordance with applicable guidelines and policies.

- At least 2 years of experience in behavioral health, social services, or human services.
- At least 2 years of case management experience.
- Must reside in Macon County, Illinois; have reliable transportation and an active driver’s license; and be able to travel 50–75% for member visits.
- Bachelor’s degree or non-licensed master-level clinician degree in behavioral health or human services (including psychology, social work, marriage and family therapy, or counseling).
Nice to have
