About the job
Conducts field-based case management for LTSS members in Northern Virginia, assessing needs, coordinating care, and connecting members with services and supports.
What you'll do
- Build relationships with members through in-person visits and conduct comprehensive assessments of medical, behavioral, and social needs.
- Develop and implement individualized care plans; coordinate with providers, community resources, and interdisciplinary teams.
- Connect members to appropriate services, advocate for medical appropriateness, and ensure care meets regulatory requirements and Aetna quality standards.
