Coordinates care for waiver and Long Term Services & Supports members in the field, including in-person visits, care planning, education, and monitoring to support appropriate healthcare outcomes.
What you'll do
- Assess members’ needs and eligibility, identify risks, and coordinate referrals and care plan activities.
- Work with members and care teams to address barriers, promote engagement, and support healthcare decisions.
- Monitor care and document services in accordance with regulatory, accreditation, and company requirements.
