Case Management Coordinator - Field Illinois (Will, Kankakee and Grundy Counties))

What you'll do

• Evaluate member needs and eligibility, identify risks, and refer members to clinical case management or crisis intervention when appropriate.
• Coordinate care plan activities, monitor progress, and collaborate with case managers and other professionals to address barriers and quality-of-care concerns.
• Coach and support members in accessing services, engaging with providers, and making informed healthcare and lifestyle choices.

What they ask for

• 2+ years of experience in Behavioral Health, Social Services, or an appropriate related field equivalent to program focus.
• 2+ years of electronic documentation experience and Microsoft Office applications.
• Willingness and ability to travel 50–75% in Will, Kankakee, Grundy, and surrounding areas; reliable transportation required.
• Bachelor's degree or non-licensed master's-level clinician degree in Behavioral Health or Human Services (Psychology, Social Work, Marriage and Family Therapy, or Counseling).

Nice to have

• Case management and discharge planning experience.
• Managed Care experience.

Pay

$21.10 - $44.99 USD base hourly rate
