What you'll do
- Evaluate members’ needs and eligibility, identify risks, and refer to clinical case management or crisis intervention when appropriate.
- Coordinate care plans, consult with multidisciplinary teams, and help members access appropriate services and make informed health decisions.
- Monitor care and document work in compliance with regulatory, accreditation, and company requirements.
