Coordinates member case-management activities by assessing needs, arranging access to care and benefits, supporting care plans, and helping members achieve appropriate healthcare outcomes.
What you'll do
- Evaluate member needs and eligibility, identify risks, and refer members to clinical case management or crisis intervention when appropriate.
- Coordinate and monitor care-plan activities; work with care teams to address barriers and escalate quality-of-care issues.
- Coach and support members in accessing services and making informed healthcare and healthy-lifestyle decisions; document care in line with applicable guidelines and policies.
