Coordinates case management for members through assessments, care planning, referrals, and follow-up to support wellness and address medical and social needs.
What you'll do
- Assess members’ medical needs, functionality, work capacity, and benefit eligibility using interviews, clinical tools, and claims information.
- Develop and coordinate care plans, referrals, and transitions to relevant clinical resources and programs.
- Present cases for multidisciplinary review and follow case management policies and procedures.
