About the job
Coordinates member case-management activities to help address healthcare and social needs, support care plans, and facilitate appropriate healthcare outcomes.
What you'll do
- Evaluates member needs and eligibility, identifies risk factors, and refers members to clinical case management or crisis intervention when appropriate.
- Coordinates and monitors care plan activities, supports member engagement and healthcare decision-making, and helps resolve barriers to care.
- Documents care and follows case-management and quality-management processes in accordance with applicable guidelines and company policies.
