Case Management Coordinator - Field (Winnebago, Boone and Jo Daviess Counties, IL))

What you'll do

• Evaluate members’ needs and eligibility, identify risks, and refer members to clinical case management or crisis intervention when appropriate.
• Coordinate and monitor care-plan activities; collaborate with care teams to address barriers and improve quality of care.
• Coach members, support healthcare decision-making, and document care in accordance with applicable processes and guidelines.

What they ask for

• 2+ years of experience in Behavioral Health, Social Services or an appropriate related field equivalent to program focus.
• Willingness and ability to travel 50–75% in the listed Illinois counties; reliable transportation required.
• 2+ years of electronic documentation experience and Microsoft Office applications.
• Bachelor's degree or non-licensed master level clinician in Behavioral Health or Human Services (Psychology, Social Work, Marriage and Family therapy, Counseling).

Nice to have

• Case management and discharge planning experience.
• Managed Care experience.
