What you'll do
- Assess members’ clinical, benefit, and social needs; identify appropriate services and care strategies.
- Coordinate and monitor plans of care with members and care teams, including crisis intervention and follow-up.
- Support member engagement, evaluate care quality, document case management, and escalate quality-of-care issues.
