Provides clinical nursing care in a value-based primary care setting, including assessments, chronic condition management, patient education, and care coordination.
What you'll do
- Triage patients, conduct clinical assessments, and perform skilled nursing procedures and onsite testing.
- Manage chronic conditions, medications, abnormal results, and patient education using established protocols.
- Coordinate care with providers and the interdisciplinary team, including hospital-to-clinic transitions.
