About the job
Completes in-home and related health evaluations while mentoring new and active Signify Health clinicians and supporting clinical workflow and service changes.
What you'll do
- Perform in-home, focused, and video visits; meet stated capacity and visit-completion targets.
- Guide new clinicians during their first 90 days and provide group and real-time support to active clinicians.
- Test new diagnostic and preventive services and clinical workflow changes, and share feedback.
