About the job
Completes in-home and other member visits while mentoring new and active Signify Health clinicians and supporting clinical workflows.
What you'll do
- Perform In-home Evaluations, Focused Visits, and Video Visits, meeting clinical, operational, and member-experience targets.
- Mentor new clinicians during their first 90 days and provide ongoing individual and group support to active clinicians.
- Test diagnostic and preventive services and workflow changes, provide feedback, and escalate clinical or operational issues as needed.
