What you'll do
- Connect patients with benefits and local resources, including housing, food, and transportation; manage referrals with the care team.
- Coordinate communication among patients, caregivers, providers, and community organizations, including outreach and appointment accompaniment.
- Document patient interactions in EPIC and participate in interdisciplinary care meetings; spend about 50% of time outside the clinic.
