Community Health Worker

What you'll do

• Coordinate patient referrals, care plans, and communication among patients, caregivers, providers, and community organizations.
• Connect patients to local resources, help with benefit applications, and support patient-defined goals and appointment engagement.
• Provide culturally appropriate health education and document patient interactions in the electronic medical record while maintaining confidentiality.

What they ask for

• At least 1 year of experience in healthcare, community-based work, case management, or a social service environment.
• Strong oral and written communication skills and ability to manage multiple priorities.
• Proficient computer skills; access to reliable transportation and ability to travel throughout the community.
• US work authorization.

Nice to have

• Community Health Worker certification or an associate’s or bachelor’s degree in a related field.
• Bilingual English/Spanish or another language commonly spoken in the community; experience with multidisciplinary teams, community resources, or electronic medical record systems.

Pay

$18.50 - $31.72 (base hourly rate or base annual full-time salary; the posting does not specify which applies to this position).
