Connects patients with care teams and community resources, advocates for patients, and helps remove barriers to health and wellness.
What you'll do
- Coordinate referrals and care plans with patients, Medical Social Workers, and community partners.
- Connect patients to local resources, assist with benefit applications, and provide culturally appropriate health education.
- Conduct in-person and phone outreach, support care team decisions, and document patient interactions while maintaining HIPAA standards.
