What you'll do
- Coordinate patient referrals and communication with care teams, caregivers, providers, and community organizations.
- Connect patients with local resources and benefits, support goal setting, and provide trained, culturally appropriate health education.
- Document patient interactions in the electronic medical record and participate in interdisciplinary care team meetings.
