Acts as a liaison among patients, community organizations, and medical teams, connecting patients with care and community resources to address barriers to health.
What you'll do
- Coordinate referrals and patient care plans with Medical Social Workers and the care team.
- Connect patients with local resources, assist with benefits applications, and provide culturally appropriate health education.
- Conduct outreach and appointments in community locations, document interactions in the electronic medical record, and participate in interdisciplinary meetings.
