Connects patients with care teams and community resources, advocates for patient needs, and helps address barriers to health and wellness.
What you'll do
- Coordinate patient referrals, care plans, and communication among patients, care teams, and community partners.
- Connect patients with local resources and assist with benefits applications, goal setting, and appointment engagement.
- Provide culturally appropriate health education and document patient interactions while maintaining confidentiality.
