About the job
Provides field-based and telephonic care coordination for West Virginia Medicaid members by assessing needs, addressing barriers, and connecting members with healthcare providers and community resources.
What you'll do
- Visit members and help with community resources, appointments, applications, transportation, and health education.
- Coordinate with care teams, providers, agencies, and community organizations; identify care gaps and arrange interventions.
- Document outreach and care coordination in the electronic health record while protecting member confidentiality.

- 2+ years of relevant experience in healthcare, community-based work, social services, case management, or similar.
- Complete an approved CHW certification program within 12 months of hire.
- Computer and MS Office proficiency; reliable personal transportation, active driver’s license, vehicle insurance, and ability to travel 70% or more.
Nice to have
- Community/outpatient or Medicaid-population experience and knowledge of local resource networks.

Pay
$18.50 - $31.72 USD base hourly rate; additional bonus, commission or short-term incentive program may be available.
