Provides clinic-based nursing care focused on assessment, chronic condition management, patient education, and coordination to improve outcomes and prevent avoidable hospitalizations.
What you'll do
- Assess and triage walk-in and telephonic patients, including reviewing abnormal laboratory results and prioritizing care.
- Provide nursing procedures, chronic condition management, medication reviews, testing, and patient and caregiver education.
- Coordinate care plans and hospital-to-clinic transitions with providers and the interdisciplinary team.
