Conduct in-home and virtual health evaluations for health plan members, reviewing medical history and medications, performing physical evaluations, and providing indicated diagnostic and preventive services.
What you'll do
- Conduct in-home health evaluations and virtual visits as needed, including medical history and medication reviews and physical evaluations.
- Provide indicated Diagnostic & Preventive Services, such as spirometry and diabetic eye exams.
- Travel locally and within licensed states as needed, and help connect members with appropriate care.
