Conduct in-home and virtual health evaluations for health plan members, reviewing medical history and medications, performing physical evaluations and indicated diagnostic or preventive services, and helping connect members with appropriate care.
What you'll do
- Conduct in-home and virtual health evaluations, including medical and medication reviews and physical evaluations.
- Provide indicated Diagnostic & Preventive Services, such as spirometry, diabetic eye exams and bone density screening.
- Travel locally and within licensed states as needed, and help connect members with appropriate care.
